Social and Emotional Wellbeing
Our Aboriginal Health Worker (Social & Emotional Wellbeing) provides support to patients.
The positions’ primary objective is to establish links with Aboriginal communities, to promote the importance of improving health outcomes and encourage and support the increased use of Durri ACMS and the broader health services system more broadly within the community.
Social wellness refers to the relationships we have and how we interact with others. Our relationships can offer support during difficult times. Social wellness involves building healthy, nurturing, and supportive relationships as well as fostering a genuine connection with those around you.
Eligibility.
Our service is available to registered patients of the Medical Service who have undertaken an Aboriginal Health Assessment (715) by a General Practitioner at Durri Aboriginal Corporation Medical Services within the past 12 months. A Mental Health Care Plan should also be undertaken by a General Practitioner at Durri Aboriginal Corporation Medical Service, please make an appointment with one of our General Practitioners (GP’s) for a referral.
Assessments and Referrals.
Internal referrals via General Practitioner, Psychiatrist, or other internal clinical and allied health professionals, send intrim mail through patient file.
External Providers – both government and community organisations
Self-referral
Counselling.
The service provides a whole family approach across all life stages. The service targets multiple and complex presenting issues:
• Mental illness, emotional distress, diagnosis (moderate)
• Family violence
• Problematic alcohol and/or other drugs
• Intergenerational trauma
• Other identified trauma – (i.e. sexual assault, child abuse & neglect)
• Disconnection with culture/identity/country
Community Awareness Programs.
Men’s, Women’s, Children Groups.
School based education sessions.
Community based group Centre.
Social & Emotional Wellbeing Hours.
Monday – Friday: 8:30am – 5:00pm
