Targeted Early Intervention – Family Support Services
The Target Early Intervention Families First program is to provide support and resources to families with young children, especially who are at risk of facing significant challenges. The program aim is to promote early childhood development, improve family functioning, and prevent issues that could escalate and affect the child’s wellbeing. TEI offers services such as parenting support, educational resources, and access to community services to help families address challenges early and effectively.
Eligibility
Our service is designed to work specifically with registered patients who require early family intervention. We provide tailored assistance to address individual needs and ensure timely intervention, aiming to enhance overall family outcomes. By focusing on proactive measures and personalised care, we strive to offer comprehensive support that meets the unique requirements of each family.
Assessments and Referrals
Durri ACMS implements a comprehensive approach to assessment and referrals to ensure that families receive the appropriate support tailored to their needs. We receive both internal and external referrals, involving various professionals and services to provide a holistic response to each family’s case. Each case is assessed individually, with a priority given to families with the highest needs. Currently, there is a caseload of up to eight families at a time, with needs spanning from minimal to significant the age ranges of children we work with is from 0 to 16.
